Issuance of Civil Service Certificate of Eligibility for No and Lost CoE

This post is going to discuss about the steps and tips on how to claim your Certificate of Eligibility for the first time and how to obtain another copy of lost CoE.

Those who will claim the certificate of eligibility for the first time, we strongly recommend for you to coordinate with the CSC Regional Offices or Field offices first to inquire on the specific dates of the availability of  your original Certificate of Eligibility (CoE). It could not be issued until further notice.

Please check the Directory of CSC Regional and Field Offices for the contact information.
These are the following requirements, you must bring in claiming your CoE: 

1. At least one valid (not expired) Identification Card (with your name, picture, date of birth and signature preferably the one presented at the time of filing of application);
 Note: ID cards accepted are: Driver’s License, SSS ID, GSIS ID, PhilHealth ID (must contain the holder’s name, picture, signature and PhilHealth number),   current Company/Office ID, School ID (must be validated for the current school year/semester/trimester), Postal ID, BIR ID (ATM type/TIN card type with picture), Barangay ID, Voter’s ID, Passport, PRC ID, and Police Clearance. Any other ID card not included in the above list shall NOT be accepted.
2.  Application receipt, if available; and
3. Two (2) identical photos similar to the picture you pasted on the Picture-Seat Plan (PSP) during the examination.

These are the 30-minute transactions in claiming your Certificate of Eligibility:


1. You must fill-out the Client Log Book first then wait your name to be called.


2. Second, present your valid I.D. and Picture with complete name tag (preferably the same as the picture used during examination ). After evaluating your requirements by the CSC personnel, you are ask to indicate the serial number of CoE and sign the copy of the Register of Eligibles.


3. Then paste your picture and affix your signature on the space provided in the Certificate of Eligibility. The CSC personnel will make a photocopy of CoE together with your valid ID then they will ask you to sign as a proof of receipt and affix the official seal of CSC on the original copy of the Certificate. 


4. Accomplish the E-6 form or known as Profile of Eligibles given by the CSC personnel.


5. Lastly, give back the accomplished E-6 form to the CSC personnel as they released your Certificate of Eligibility.


NOTE: According to CSC you must PERSONALLY claim your COE.

Those who lost their certificate due to typhoon, flood, fire and theft, who did not claim and receive their certificate, who submitted their certificate to any agencies and who want to replace their certificate because it is old, torn, worn-out, and requesting for correction of personal information, please be advised to bring the following requirements:

1. Accomplished Eligibility / Examination Records Request Form (ERRF) and Delartion Form (DF)

2. One (1) copy of 1x1 picture with name tag and signature affixed prior to having the photograph taken.
  • Signature must be on top of the printed name
  • Photograph should have been taken within three (3) months prior to filing of request for Certification of Eligibility
  • Scanned, computer-generated photo/name/signature will not be accepted.

3. Certification fee of Php 100.00 per copy
4. Two (2) valid identification (ID) Cards (valid ID contains applicant's clear picture, date of birth, signature, and signaure of the authorized head of the issuing agency such as Driver's License, SSS ID, GSIS ID, PhilHealth ID, current Company/Office ID, current school ID, Postal ID, BIR ID, Barangay ID, Voter's ID, Valid Passport or Police Clearance. Any ID not included in the list shall NOT be accepted) 

or


If the requesting party works/lives abroad:

1. Copy of Passport
2. Copy of one (1) Identification Card 
-both are duly authenticated by the Philippine Embassy or Consular Office

Special Requirement:

1. NSO-Issued Marriage Contract for women who married after taking the examination

Additional requirements if request is filed through a representative:

1. Authorization Letter or Special Power of Attorney (SPA)
2. One (1) valid Identification Card of the Representative

1. Fill-out Eligibility/Exam Records Request Form (ERRF (Revised 01192011)) and Declaration Form (DF) given by the CSC personnel.

2. Wait for the request to be processed
3. The CSC personnel will issue order of payment and you are advise to pay at the cashier
4. Pay to the cashier and receive the Official Receipt.
5. Present OR and affix signature on the release portion of the ERRF or file copy as the CSC personnel releases your Certificate of Eligibility.

  1. Hello, question about this, Can I use my NBI Clearance as an identification?I don't have any valid ID as of the moment and My school ID is a alumni ID. can i use that one instead? please respond, thank you.

    I just want to ask about the best way for a polio victim to go about getting a certificate of exemption for Civil Service Exam , since I graduated with honors. I would like to try and apply to serve our country more but they are asking me to get this certificate as part of the requirements.
    If there is a portal or site where I can submit all my credentials as I am not that agile to get around because of my leg disability.

  5. good day po. nakapag take po ako ng CSE-PPT sa region 4-A sa dasmarinas last April 2014, pero andito po ako ngaun sa manila. pwd ko po ba makuha yung certificate of eligibility ko sa inyong National/ Main office?

  6. Hi po. Magandang araw po. I took the Civil Service Exam-Professional last May 3, 2015. Okay lang bang iclaim yung certificate ko kahit na wala yung application receipt ko?. Nakalagay naman dun sa requirement na need ang application receipt if available. I think, napasama sya sa mga nabasa kong gamit nung nasalanta ng bagyo ang oriental mindoro.please,sana po magreply kayo sa akin or you could send your reply through my gmail (alcantarakriziamaan@gmail.com)

Carnicer, i took the professional civil service exam way back year sometime int he year 1995 or so, fortunately i passed and had been issued the corresponding certificate, and that was lost during typhoon Pablo that badly hit our place last December 4, 2012...Can i have my new certificate? ??...kindly send your reply thru gmail (risenrock20@gmail.com)

  9. Tanong lang po, sa Batasan Branch po ako nakapasa ng exam. Pwede po ba na sa Tuguegarao Branch ako kumuha ng Authentication Eligibility ko? Kailangan ko po kase sa promotion. Thank you po.

  11. What if I lost po the picture na sinulatan sa likod noong examination day pero may copy pa po ako pero ung walang sulat po. Okay lang po ba un? Similar photo naman po.

  12. Hi. Ask ko lang po f my prescriptive period ung pagclaim ng certificate of eligibility.?,i took the exam during oct2014 but for various reasons, i was't able to claim it. Thanks.

  13. Can I still claim my certificate 2 years after taking the exam? I already submitted required documents, however, the staff told me that I can't claim my certificate due to different place of birth reflected in their system vs in my birth certificate. I presented my school i.d and birth certificate at that time and told me to first file an application to change data before they release my certificate. Thanks.
