Schedule of Release of the CSC Eligibility Card for 2015 CSE-PPT and FOE Passers

The Civil Service Commission (CSC) has announced through Examination Advisory No. 32, s. 2017 the schedule of release of the CSC Eligibility Card for the August 16, 2015 Fire Officer Exam, October 18, 2015 and December 6, 2015 CSE-PPT passers.

TO : All Passers of the August 16, 2015 FOE, October 18, 2015 CSE-PPT Professional and SubProfessional Levels, and December 6, 2015 CSE-PPT Professional and SubProfessional Levels

SUBJECT : Schedule of Release of the CSC Eligibility Card

Please be advised that passers of the August 16, 2015 Fire Officer Examination, October 18, 2015 Career Service Examination, Paper and Pencil Test (CSE-PPT), Professional and SubProfessional levels, and December 6, 2015 CSE-PPT, Professional and SubProfessional levels may now personally claim the CSC Eligibility Card at the CSC Regional Office where the eligible took the examination, except for the CSC Regional Office No. IV and CSC National Capital Region, beginning December 18, 2017.

Representatives are not allowed to claim the card on behalf of the passer. Concerned passer is required to present to the releasing officer the following requirements below upon claiming of CSC Eligibility Card.

1. Any of the following I.D. cards, which must be valid (i.e. not expired on the date of claiming):
· Driver’s License;
· Passport;
· PRC License;
· SSS I.D.;
· GSIS I.D. (UMID);
· Voter’s ID;
· BIR ID (ATM type/TIN card type with picture);
· PhilHealth ID (must, at the least, contain the holder’s name, clear picture, signature and PhilHealth number);
· Company/Office ID;
· School I.D. (validated for the current school year/semester/trimester);
· Police Clearance/Police Clearance Certificate;
· Postal I.D.;
· Barangay I.D.; or
· NBI Clearance.
Note: All other I.D. cards shall not be accepted

2. Certification of Eligibility printed on CSC letterhead (if available); and

3. Examination Application Receipt and/or CSC Official Receipt (if available).
Note: The CSC Regional Office may require additional document/s as necessary to establish the identity of the passer.

The passer availing of the CSC Eligibility Card shall pay an application fee amounting to Php 200.00.

In case of claim of the CSC Eligibility Card from outside the jurisdiction of CSC Regional Office/Field Office where the passer took the examination, the concerned passer may request the same in writing (through mail, courier, facsimile, or e-mail) addressed to the Regional Office where the passer took the examination, citing reasons for the request to release the card at the nearest CSC RO or CSC FO, and wait for the reply/instruction of concerned Regional Office.

Passers of the CSC Regional Office No. IV and CSC National Capital Region for the October 18, 2015 and December 6, 2015 CSE-PPT Professional and SubProfessional levels may initially coordinate with the said CSC Regional Offices on the availability of the CSC Eligibility Cards before claiming.
